Speak to the STOP HIV/AIDS Outreach Team for case management and primary care support for HIV positive patients who are newly diagnosed or have difficulty engaging in care. Services include: outreach, ARV treatment initiation, help with housing or disability forms, blood work, immunizations and ARV adherence supports.
